Report an Incident Immediately

Departments must report incidents as soon as possible to ensure timely processing and proper documentation. Delayed reporting can affect claim eligibility.

Liability Claims

If an incident involves injury to a non‑employee, property damage to others, or potential legal exposure:

  • Complete a General Liability Incident Report Form.
  • Provide supporting documents (photos, witness statements, police reports if applicable).
  • Submit the General Liability Incident Report Form and any supporting documents to the Insurance and Claims team, via email at gtinsurance.ask@business.gatech.edu.

Automobile Claims

For accidents involving state‑owned, leased, or rented vehicles:

  • Ensure everyone is safe and contact law enforcement if required.
  • Notify your supervisor.
  • Use the Auto Liability Identification Card at the scene.
  • Report the incident via email at gtinsurance.ask@business.gatech.edu using the applicable auto claim form and provide required documents (police report, photos, statements).
